Create user activity logs

Sometimes a user, a reseller or even managed server clients calls and asks me when mx records has changed while we did not change anything.
(mx is just an example)
We can never really find who changed that and when and often the hoster gets the blame.
Having user activity logs would be a great addition.
We would like to see what a user did and when.
Looking back at what pages he visited is not enough since the actual change seems not to get logged.

Not really.
You can see if a user has logged in.
It is not possible to see what that user did while logged in.
For instance: You cannot see if a user removed files, replaced files, uploaded files, changed PHP version etc.
